WHAT IF THE LONG-TERM CARE FACILITY ASKS ME TO MOVE?
If a Long-Term Care Facility asks you to move, you can only be moved if:
The home cannot meet your needs.
You no longer need Long-Term Facility Care.
The health or safety of other residents is endangered.
You do not pay for your stay.
Prior to Moving:
The Long-Term Care Facility must tell you in writing…
The reason for the move
When and where you would go
How to appeal
And that you can stay until your appeal is heard
